Module 2: Transformation Fund Application
KHVVG KHTF
New since August 26, 2026: The Federal Office for Social Security has published the funding guidelines for the Hospital Transformation Fund. This means that funding requirements, application documents, and procedures are now established. For hospitals, this means: The target structure, with which a project enters the state’s prioritization, is now being determined.
What transformation does your hospital need, and what opportunities from digitalization and AI should already be considered today?
From Need to Support – Implementing Clinical Transformation Effectively
Seizing Opportunities, Avoiding Pitfalls
The Transformation Fund opens up great Opportunities, but The path to funding is challenging. Required Are not Just strong strategies, but also robust business cases, Viable planning and the agreement of all relevant actors. Funding decisions are not solely based on speed. The states submit applications and prioritize projects; there is no legal entitlement to funding. Therefore, a project that convinces and is supported by the state is crucial. However, a time factor remains: For applications submitted by December 31, 2029, a co-financing of 30 percent is sufficient; thereafter, it increases to 50 percent.
Our Three Levers for Your Success: A Clear Location strategy, a future-proof service portfolio A smart networking and targeted performance boosters Through digitalization, infrastructure and process optimization.
More Than Just a Funding Application
The Transformation Fund is an opportunity to fundamentally rethink a facility’s entire structure: location strategy, service portfolio, care models, economic viability, processes, and digitalization. Those who treat the application as a mere formality miss the chance to shape the target structure for the next ten years.
Digitalization and AI Belong in the Target Structure, Not in the Addendum
Digital infrastructure is only eligible for funding insofar as it is necessary for the structural project. Measures that serve exclusively to modernize digital infrastructure are explicitly not eligible for funding. Those who consider digitalization and AI only after submitting the application will finance them alone later. That’s why the question of how the future care and operating model will work must be part of planning the target structure from the start.
The Advantages
Recommended Starting Point: Our KHTF Workshop and Our WMC Basic Analysis
We develop a funding strategy with you, clarify the current framework conditions and funding options.
In just a few weeks, we specifically review the medical strategy from a transformation fund perspective – data-based, structured, and realistic. Additionally, we offer our “Quick-Check Digitalization as a Performance Guarantee”, which identifies efficiency potentials along central clinical processes – often with a return on investment in less than a year. This can be applied to either the entire facility or individual areas, such as the Emergency Department.
New: the AI Readiness Check. We identify concrete AI potentials in medical and administrative processes and prioritize them according to benefit, feasibility, and strategic relevance. The result is a reliable picture of your facility’s maturity level and a prioritized list of next steps.
Prepare Strategically for Structural Change Now
Let us jointly examine which transformation, funding, and AI potentials exist for your facility.
