With the Hospital Care Improvement Act (KHVVG), a new era of inpatient care begins in 2025. At the center are nationally introduced service groups that define clear structural and quality requirements. Hospitals that do not meet these requirements or are at a competitive disadvantage will not only lose care contracts but also standby allowances and DRG revenues.

The legislator has created the Hospital Transformation Fund (KHTF) to ensure a future-proof realignment. Until 2035, 50 billion euros are available to promote investments in modern care models, site developments, and the strategic bundling of services.

Even financially well-positioned hospitals should seize the opportunity to secure KHTF funds – for example, for infrastructure, digitalization, or performance management. Many investments are only subsidized once per region, and since the final layouts of the healthcare regions are still open, waiting is not an option. Now is the time to actively manage the transformation, rather than being overwhelmed by it.

The application for service groups is scheduled in most federal states in the coming weeks and months. Therefore, hospitals must quickly decide which service groups they want to provide and ensure that they meet the minimum criteria for staffing and infrastructure. If, for example, there is a lack of specialized personnel, this can only be acquired with sufficient lead time. At the same time, funding must be applied for. Transformation incurs costs.
